NOTE FROM THE NURSE
If your child has medication and/or medical supplies remaining in the health office, they will need to be picked up by a parent/guardian. Emergency medications should remain in the office through the last day of school.
Please pick up medication/supplies on one of the following days:
June 10 from 8:45-4:00pm
June 12 and 13 from 8:45-4:00 pm
Per the Virginia Department of Education, medication CANNOT be stored over the summer.
Medication WILL BE DISCARDED if not collected by the end of day on Tuesday, June 17th.
Loudoun County Public Schools requires a new provider/physician orders each school year for the medication or specialized care which your child receives at school. If your child will continue to need medication or specialized health care at school, you and your child’s licensed healthcare provider must complete and sign the proper form(s). Please note that parents may not amend the provider’s order for managing the child’s medical condition.
The required forms are available in the health office at your child’s school or online at www.lcps.org/medicalforms. The forms should be returned, along with the appropriate medication, in the original, pharmacy labelled container, the week before school starts to the Loudoun County Public School that your child will be attending. Please refer to the “Medication at School Guidelines” for further information regarding medication at school.
If you have any questions, please contact the nurse at maryellen.foulkes@lcps.org or 540.751.2400.
