John W. Tolbert Jr. Elementary

5/28/2026

Dear Parent/Guardians:

If your child has medication and/or medical supplies remaining in the health office that need to be picked up by a parent/guardian, please pick up medication/supplies on one of the following days:

  • June 10 from 8:00 am – 2:00 pm

  • June 11 from 8:00 am - 2:00pm

  • June 12 from 8:00 am – 2:00 pm

(Emergency medications should remain in the office through the last day of school.)

If you need to schedule a pick-up time before or after the times listed above, please call the clinic at: 571-252-2870 #2.

  • Per the Virginia Department of Education, medication CANNOT be stored over the summer.

  • Medication WILL BE DISCARDED if not collected by 12:00 pm on Monday, June 15th.

Loudoun County Public Schools requires a new provider/physician orders each school year for the medication or specialized care which your child receives at school. If your child continues to need medication or specialized health care at school, you and your child’s licensed healthcare provider must complete and sign the proper form(s). Please note that parents may not amend the provider’s order for managing the child’s medical condition.

The required forms are available in the health office at your child’s school or online at www.lcps.org/medicalforms. The forms should be returned, along with the appropriate medication, in the original, pharmacy labelled container, the week before school starts to the Loudoun County Public School that your child will be attending. Please refer to the “Medication at School Guidelines” for further information regarding medication at school.

Any concerns contact Mrs. Atkins at denise.atkins@lcps.org or 571-252-2870 #2.
