Our Health Services Office is here to help our students with minor injuries, health concerns and daily medications.

If you would like copies of your child's immunization records, please notify our health services specialist in advance. We ask, as a courtesy, that you give her at least a day's notice to prepare the records for your pick up. Thank your for your cooperation.

Loudoun County Public Schools Health Services Website, all the forms you need
This website will help parents better understand the services we can provide. It will also give parents the links to the forms they may need for immunizations, TB screenings and well visits.
If you want your child to take any medications while at school, all prescription medications require a doctors order on the appropriate authorization form. (some of which are provided below) For a complete list of medical authorization forms, should your child require medication for any other condition please go to the Loudoun County Public Schools Health Services Website list above for a complete list of authorization forms. ALL medications must come in their original containers and instructions for use must match the doctors order or as directed by the label from the manufacturer. "Over the Counter" medications include but are not limited to: Tylenol, Advil,Benadryl, Cough Drops
FORMS YOU WILL NEED TO ACCOMPANY YOUR CHILD'S MEDICATION DROP OFF
- For Epipens (or prescription allergy medication) Physician's Allergy Action Plan
- For AsthmaPhysican's Asthma Action Plan
- For "over the counter" medications (such as tylenol, motrin, sunscreen, cough drops or bendadryl) parents must complete the Parent Authorization Form and bring with the medication to the health services office. Parents Authorization for Medication
