The Commonwealth of Virginia School Entrance Health Form (MCH-213 E), required by state law, may be obtained at your local school.
In order to authorize our principal's designee to administer prescribed medications to your child from your physician or any over-the-counter medications that you have brought to us for this purpose, we must have the following form on file here at our office. (Please click on link below and print form, then fill it out and return it to the school office yourself.)
DO NOT SEND ANY MEDICATION ON THE BUS.
