• VHSL Athletic Participation/Parental Consent/Physical Examination Form (Rev 2017)

    Not strictly required to join the NJROTC program, a current pre-sports participation physical is required for any cadet desiring to:
    a) take part in certain co-curricular teams (fitness, drill, color guard, marksmanship and orienteering)
    b) participate in the semi-annual cadet fitness challenge
    c) participate in certain activities during field trips or military base orientation visits.
     
    Physicals completed after May 1st are valid for the upcoming school year.  Copies will automatically be provided to the Athletic Office of any VHSL forms submitted for NJROTC students who desire to also participate in fall, winter or spring varsity sports.  All four parts of this form must be signed and filled in completely for it to be valid.  It is recommended that both this form and the Health Risk Screening Questionnaire be taken together to the doctor's office during your child's annual physical exam (ideally sometime between May 1st and July 30th) so that they can both be completed in time for new school year.

  • Record of Cadet Information

    A new Record of Cadet Information (RCI) form is required each year in order to update the contact information we have for each cadet and their parents.

  • NJROTC Standard Release Form

    A new Standard Release Form must be completed for each cadet by a parent or guardian at or before the beginning of each school year.  Both pages must be filled out.  The medical and dental insurance information on page 2 is not absolutely required, however this information may be necessary to obtain non-emergency care should the need arise, particularly when away from the school during field trips, service projects, and team competitions.  Note that a parent or guardian must sign at the bottom of page 2 and also initial in the lower right corner of page 1.  Failure to submit this form will result the student not being allowed to enroll (or being disenrolled) from the LCHS NJROTC program.

  • NJROTC Health Risk Screening Questionnaire

    A new Health Risk Screening Questionnaire must be completed for each cadet by a parent or guardian at or before the beginning of each school year.  If any items are checked "yes" in Part A, then Part B must also be completed and signed by a licensed medical doctor or registered school nurse.  Note that this form complements, but does not replace, the requirement to complete a VHSL athletic participation/parental consent/physical exam form.  Failure to submit this form will result the student not being allowed to enroll (or being disenrolled) from the LCHS NJROTC program.
