    This form came home with your child on the first day.

    If you have not signed and returned this, please print this, fill it out,

    sign it and return it to school with your child. 

    This form is required for all students using CTE equipment in a lab.  

    This agreement must be signed by both student and a parent

    prior to any student using equipment in a lab.
