    Fund Request Form 

    • To be used when staff/teacher or student wants to request funds from the PTSO 
    • Please submit to PTSO mailbox at least one week before the monthly meeting for consideration by membership at the meeting

     

    Reimbursement Request Form

    •  To be used when pre-approved expenses need to be reimbursed 
    • The above forms should be submitted to the PTSO Treasurer at the monthly meeting or dropped in the PTSO mailbox in the school mailroom.

     

    Receipt of Income

    • To be used when funds are to be deposited for the PTSO
    • The above forms should be submitted to the PTSO Treasurer at the monthly meeting or dropped in the PTSO mailbox in the school mailroom.

     

    PTSO Parent Membership Form 

    The above form can be dropped in the PTSO mailbox in the school mailroom or sent via email.  Payments can be made via paypal at https://www.paypal.me/sbhsptso
